Booking Instructions
•For Family Physician Appointments: Online booking is available only for registered patients under our family physicians.
•For Walk-In Appointments: Please use the walk-in application form instead of the online booking system.
•Appointment Availability:
Booked appointments will be scheduled 4–6 days from the current date.

The online booking link is available only for Dr. Tonycheta, Adeaze and Dr. Adedibu, Samuel.
For all other doctors, please use the Registered Patients Appointment Form.
When booking, please ensure you select the correct reason and type of appointment.
Appointments booked under the wrong category (e.g., a follow-up booked under Physicals or Pap Test) will be cancelled without notification.
